脱发是一种常见的皮肤病，可分为男性型脱发（又称雄性激素源性脱发、雄性秃，俗称早秃、谢顶或地中海等）、脂溢性脱发（又称脂秃）、斑秃等十几种。常见的 脱发大多为男性型脱发（占脱发患者50%以上）和脂溢性脱发（占45%以上） ，其它类型的脱发则总共只占脱发患者总数的5%（其中斑秃占3%，其余的约仅占2%）。有许多人包括很多医生总把脂溢性脱发与男性型脱发混为一谈，其实这 是一种混淆不清的谬误，因为脂溢性脱发除了有与男性型脱发相同的雄性激素水平异常的原因外，还具有其本身所独有的原因和特征。
脂溢性脱发 是在脂溢性皮炎的基础上发生的一种脱发，其症状为患者头皮脂肪过量溢出，常伴有头屑增多，头皮油腻，瘙痒明显。多发生于皮脂腺分泌旺盛的青壮年。患者一般 头发细软，有的还伴有头皮脂溢性皮炎症状。开始逐渐自头顶部脱发，蔓延及额部，继而弥漫于整个头顶。头皮油腻而亮红，结黄色油性痂。有些男性在发育后，即 开始出现脱发，进而头发油腻发亮，头皮屑慢慢增多，经常出现奇痒，有时头发干枯无光泽，只要用手抓一抓，头发就会脱落，特别是两侧额角还会发生慢性弥漫性 脱发。

一、脂溢性脱发临床表现：
脂溢性脱发以男性多见，且脑力劳动者多于体力劳动者，但近些年来女性患者人数有增加的趋势。临床表现为患者头皮脂肪过量溢出，导致头皮油腻潮湿，加上尘埃 与皮屑混杂，几天不洗头就很脏，并散发臭味，尤其在气温高时更是如此；有时还伴有头皮搔痒炎症，主要是由于头皮潮湿，细菌繁生感染引起脂溢性皮炎。脱发一 般先从两额角、前额和头顶中间开始，继而弥漫于整个头顶，但头部四周的头发却不怎么掉。症状严重者脱发区变得油光发亮，剩余的头发变得细软枯黄。患者一般 从二十多岁开始就出现持续性的脱发，严重者到三十多岁，一般到四十多岁就基本脱光，严重影响美容。脱发的发展进程是越来越严重，开始时每天可见许多头发落 在枕巾和发梳上，到后来只要轻轻一拔就可以拔下一大把 ，洗头时满脸盆都是脱发，浴室的出水口铺满厚厚一层的脱发，叫人心疼万分，又不胜惶恐。

二、脂溢性脱发的分类：
脂溢性脱发分两类：急性脂溢性脱发和慢性脂溢性脱发。
（1）急性脂溢性脱发 症状是：头皮油质增多、痒，有头屑或丘疹，毛发在短时间内成撮脱落甚至全部脱光，头皮可有小丘疹，多发生在青春期，以男性较多见，治愈后易复发。
（2）慢性脂溢性脱发 症状是：男性多见头皮油腻发亮，呈涂油状，有大量灰白色糠皮状头屑，头发干燥，缺乏光泽，瘙痒较重，男性头发从前额两侧及头顶部慢慢脱落，几年或十几年后 形成秃顶，但不易形成全秃。 而女性则是表现为头发稀少干枯，毛发也是慢慢的、散在的脱落，露出头皮，但很少有形成秃顶的可能。

三、脂溢性脱发的原因：
长期的研究与观察认为导致脂溢性脱发最直接的关键原因在于脂溢性皮炎，皮肤中有些成份如油酸、亚油酸、角鲨烯等过量时对毛囊有毒性作用，导致毛皮中毒、枯 萎、脱落。实验表明，将含有这些成份的油脂涂到动物皮毛上就会引起皮毛的大量脱落。脂溢性脱发与脂溢性皮炎有直接关联的证据之一是，人体皮脂分泌与气温高 低有关，气温越高皮脂分泌越多。故在夏秋高温季节脂溢性脱发就特别严重，在冬春季症状会轻一点，这一般患者都有体会 ；这一点用体内血清雄性激素水平的高低是无法解释的，一般而言雄性激素在体内的水平是稳定的，与气温高低并无太大关系。
故目前尚无法确认雄性激素过多是脂溢性脱发的根本原因。另外，传统中医认为脱发是由肾血亏虚、肝郁气滞、阴虚内热引起。但这种看法对各种不同类型的脱发缺乏明确的分类和细致的病理分析。应看到各种不同类型脱发的病因是不一样的，很难全都归因于肾虚，尤其是脂溢性脱发。

四、脂溢性脱发是怎样形成的？
从事紧张复杂 脑力劳动的人，精神压力大且大脑消耗能量极大，刺激机体自主神经和激素适应性的作出调整，使人的性情变得较为急燥，皮脂腺分泌旺盛。头皮上的皮脂腺同其它 部位相比是最发达，在这种情况下分泌就更多，从而为头皮上嗜脂性真菌及头螨等的大量繁殖提供了条件。头皮上的嗜脂性真菌在皮脂腺分泌量大的前提下大量繁 殖，成灾的嗜脂性真菌从毛囊中获取营养并把代谢产物排放在那里，刺激毛囊和头皮出现慢性炎症－脂溢性皮炎， 脂溢性皮炎如得不到及时治疗，发根部细菌生长繁殖产生一种溶解酶，将发根溶解成残缺不全，发根松动，毛囊逐渐萎缩，生发功能逐渐减退，头发逐渐减少直至光 秃。 头螨是一种微小的寄生虫，肉眼无法看见，毛发专家已经发现，头螨这种嗜脂性微小动物，和遗传或荷尔蒙失调一样，也是脱发的元凶。它寄生在人类的毛囊里，以 皮脂为食物，吃饱之后为了帮助消化，它会分泌一种解脂酵素（lipase），这种酵素会进一步分解和侵蚀头皮内的皮脂腺，阻塞毛囊，令毛囊缺乏养分而萎 缩。

乾癬之誘因

一、遺傳：父母有一人患乾癬，則10%子女會得此病；雙親皆有乾癬，則30%子女可能罹患本病。
二、內分泌影響：青春期及更年期發病率較高。女性患者在懷孕時病情會改善。故推論本病與內分泌有關。
三、外傷：洗髮時不正確，指甲過長抓傷導致細菌倍增
四、感染：經上呼吸道感染或扁桃腺發炎數週後，可誘發此病。
五、化學物品使用不當可誘發此病。
六、情緒緊張或過度身心疲勞可誘發或使病情加重。
七、日光曝曬不當可誘發此病。

乾癬之臨床表現

一、皮膚：部位：多侵犯身體四肢伸側(尤其在肘、膝關節)、頭皮、胸骨、肩胛骨、薦臀等部位，而臉部則較少見。病灶：界限明顯、紅色脫屑之丘疹或 斑塊，表面常覆蓋銀白色鱗屑。癢感：病灶可能劇癢。

二、指甲：呈凹陷、變黃褐色、粗糙、肥厚、鬆動、脆裂或脫落。

三、關節：約7%之病患可能合併關節病變。乾癬之治療(方式很多但需長期治療)

乾癬患者之日常保健

一、癢之處理：1.穿著寬鬆棉質之衣物。2.輕拍癢處，不可用力抓癢 否則會導致壞細菌滋長更嚴重 因為細菌滋長是屬於倍增。 3.可到髮廊去做免費頭皮檢測 對症下藥! 4.按時修剪指甲以避免抓傷皮膚。

二、飲食方面：須避免吃刺激性食物，如煙、酒、辣味等。

三、保持情緒平穩及正常規律生活，避免熬夜及過度疲勞。

四、預防上呼吸道感染或扁桃腺發炎。

五、適當日光照射：上午九點前及下午三時後之陽光，以免灼傷皮膚。

六、本病不會傳染，不須與家人隔離。

七、不可口服或注射皮質類固醇(美國仙丹)，以防病灶惡化。

Gray Hair – Why Does Hair Turn Gray?

Each hair on our heads is made up of two parts:

• a shaft – the colored part we see growing out of our heads
• a root - the bottom part, which keeps the hair anchored under the scalp

The root of every strand of hair is surrounded by a tube of tissue under the skin that is called the hair follicle (say: fah-lih-kul). Each hair follicle contains a certain number of pigment cells. These pigment cells continuously produce a chemical called melanin (say: meh-luh-nin) that gives the growing shaft of hair its color of brown, blonder, red, and anything in between.

Melanin is the same stuff that makes our skin’s color fair or darker. It also helps determine whether a person will burn or tan in the sun. The dark or light color of someone’s hair depends on how much melanin each hair contains.

As we get older, the pigment cells in our hair follicles gradually die. When there are fewer pigment cells in a hair follicle, that strand of hair will no longer contain as much melanin and will become a more transparent color – like gray, silver, or white – as it grows. As people continue to get older, fewer pigment cells will be around to produce melanin. Eventually, the hair will look completely gray.

People can get gray hair at any age. Some people go gray at a young age – as early as when they are in high school or college – whereas others may be in their 30s or 40s before they see that first gray hair. How early we get gray hair is determined by our genes. This means that most of us will start having gray hairs around the same age that our parents or grandparents first did.

Gray hair is more noticeable in people with darker hair because it stands out, but people with naturally lighter hair are just as likely to go gray. From the time a person notices a few gray hairs, it may take more than 10 years for all of that person’s hair to turn gray.

事件一：少女用染发剂不当 头部过敏像外星人

2007年4月据英国每日邮报报道，英国一位十几岁少女使用染发剂之后出现皮肤过敏，面部竟出现严重过敏肿胀，按她的话说自己看上去就像外星人。

生活在伦敦南部莫汀汉姆地区的13岁洛伊斯奎恩在使用褐色XXX牌染发剂之后，感觉头部疼痛，眼睛酸痒，立即到医院接受治疗。苦恼不安、以泪洗面的奎恩自使用完染发剂之后面部肿胀已有10天时间，目前，她发誓从此再也不染发。据了解，奎恩是在家附近一家商店花5.99英镑购买这瓶染色剂，她希望把自己的头发染一下然后庆祝5月份14岁生日。

奎恩仔细看过包装说明，使用时她的母亲洛纳一直在旁注视她的使用情况，奎恩在使用前还用敏感试片测试了48个小时。然而使用后让她非常苦恼，自己的头部肿胀得气球，她急忙到医院就诊，医生称这是严重的过敏反应，并给她开了类固醇和抗组胺剂。

奎恩说，“我感觉到强烈的疼痛，当时我感到非常恐惧，使用之后几个小时眼睛就变红发痒，然后开始肿胀。第二天早上我几乎不能睁开眼睛。我认为自 己可能会失明或导致死亡，我的头部呈现出怪异的形状，照镜子看自己非常像外星人。”她指出，医生给我的药物并没有转好，反而逐渐恶化，我不得不再次去医院 三次。据悉，奎恩已一个星期没有去学校上课，这段时间是她最难熬的，她在家中没有食欲进食，也从不出家门。

事件二：“欧莱雅”惹祸 英国一少女染发变“象脸”

2006年5月据英国当地媒体报道，英国布里斯托尔31岁的塞勒克购买了一支XXX彩滋养染发霜（XXX Nutrisse）。在他染发后 不久，便感到头皮很痒，然后开始觉得炙痛，1小时后头部开始肿胀。医生为塞勒克开了治疗过敏的药物“抗组织胺药”。然而，服了医生开出的药后，塞勒克的情 况反而恶化，用塞勒克自己的话说：“我尝试戴上棒球帽，免得别人见到我当时的模样，但我的头肿得连帽子也戴不上。”塞勒克这种头大如瓜的情况维持了整整7 天时间。

44岁的达顿在使用了一支XXX红色染发霜后，同样“中招”，“染发后我的头部和脸部变得肿胀。”此后，达顿被送进医院抢救6个小时。

英国格兰瑟姆21岁的男子佩里使用XXX染发霜后也出现头部肿胀。佩里投诉说：“染发几天后，我开始发疹和出水泡。接着头部开始肿胀。”他现在正考虑是否控告欧莱雅的旗下品牌卡尼尔。

据悉，2004年10月，英国伯明翰夫人德维用了XXX附属公司雅恋推出的“XXX”染发剂，结果出现过敏性休克，其喉部及面部发胀，气管被阻塞，最后死亡。该产品还造成10多人皮肤严重灼伤。

目前XXX公司正因此面临来自英国及丹麦的多项诉讼。

屡次发生类似的事件，世界知名大品牌的XXX集团作为当事人又是如何回应的呢？

事件一：

XXX中国表示，由于此事件是发生在海外的单一案例，与中国市场无关，因此XXX中国暂不准备就此发表声明。“我们不认为我们的产品存在质量问题。”———这位负责人称。

XXX中国总部公共事务部周根良经理则表示，在英国出现不良反应的是XXX营彩滋养染发膜系列产品中的一个品种“100％”，目前这一品种还没有在中国销售。他们也没有接到国内消费者对XXX染发剂任何产品的投诉。

事件二：

事故发生后，XXX产品发言人表示：“欧莱雅的产品是绝对安全的，我们了解了奎恩小姐的状况，决定对她进行医疗赔偿。”发言人表示：“像奎恩这 样的过敏例子非常少见，但我们不否认确实会有这样的个例。消费者在使用染发产品之前一定要做皮肤过敏测试，48小时后没有出现过敏反应才可以安全使用”。

事件三：

XXX北京分公司的公关处经理陈瑶说，“对苯二胺”是染发剂产品中常见的有效成分，不是国际禁用的化妆品成分。XXX染发产品经过国际和中国相 关部门的严格检验，产品中含有的成分都是完全符合中国有关规定的。“对苯二胺”对普通人是没有任何副作用的，只有极少数对其过敏的消费者使用后可能会产生 过敏反应。

三，事件分析——酿成惨剧 到底谁之过？

染发剂原料的潜在危险

染发剂，顾名思义就是给头发染色的一种化妆品。它还分为：暂时性染发剂，半永久性染发剂，永久性染发剂三大类别。

据介绍，大多数染发剂中含有重金属铅以及其他对人体有害的化学物质等。《化妆品卫生规范》对这些有毒物质规定了一定限量，同时还规定了一些成分禁用在化妆品中，如不得含有甲苯胺类及其同分异构体、盐类以及卤化和磺化衍生物等成分。

市场上的主流产品，它不含有一般所说的染料，而是含有染料中间体和偶合剂，这些染料中间体和偶合剂渗透进入头发的皮质后，发生氧化反应、偶合和 缩合反应形成较大的染料分子，被封闭在头发纤维内。由于染料中间体和偶合剂的种类不同、含量比例的差别，故产生色调不同的反应产物，各种色调产物组合成不 同的色调，使头发染上不同的颜色。由于染料大分子是在头发纤维内通过染料中间体和偶合剂小分子反应生成。因此，在洗涤时，形成的染料大分子是不容易通过毛 发纤维的孔径被冲洗。

有过烫发经历的爱美人士都有这样的经历:在烫发的时候，药水多多少少会接触到头皮，这时会伴随着强烈的灼痛感，而且还带有刺鼻的臭味。烫发剂对人体有害吗？烫发剂是否也含有对苯二胺或者其他有害物？

染发剂普遍含有对苯二胺这种致癌物质，近年来，染发俨然已成为人们的时尚选择，据央视国际网络调查显示，在接受调查的2600多人中，染过发的人占到了90%以上，而且在30岁以前开始染发的人占到了被调查者的半数左右。

染发剂接触皮肤，而且在染发的过程中还要加热，使苯类的有机物质通过头皮进入毛细血管，然后随血液循环到达骨髓，长期反复作用于造血干细胞，导致造血干 细胞的恶变，导致白血病的发生。而染发剂之所以会导致皮肤过敏、白血病等多种疾病，是因为染发剂中含有一种名叫对苯二胺的化学物质。专家称，对苯二胺是染 发剂中必须用到的一种着色剂，是国际公认的一种致癌物质。

染发产品致癌

尽管有研究报道发现染发与某些癌症存在一定相关，但国际癌症研究机构曾对染发与癌症关系进行过评估，认为没有充分证据证明消费者染发致癌。

染发产品致敏

多种染发产品原料都具有一定的致敏作用，染发引起的过敏反应与消费者的机体敏感程度有关。根据消费者机体敏感程度不同，致敏反应的轻重不一，可能是局部的，也可能是大范围的。容易发生不良反应的是耐久性染发剂，能通过皮肤被人体吸收，极易出现过敏性皮炎、湿疹、鼻炎、哮喘、恶心、呕吐、头痛等症状。因此，属过敏体质的人，最好不要使用这种染发剂。

皮肤检测很重要

在市内多家大型百货商场欧莱雅专柜均发现了纷莹染发产品，在该产品的中文说明上印有“该产品可能会引起过敏反应，而在极少数的情况下会有严重的 过敏情况发生，使用前48小时请做皮肤敏感性测试”的警告字句。在翠微大厦，销售人员告诉记者，这款产品卖得不错，“平均一天能卖七八盒，没听说出过什么 问题。”

据了解，全球最大化妆品公司欧莱雅目前在中国销售的家用染发产品共有3款，分别是：卓韵、纷莹和营彩系列，另外还有用于专业发廊的今年8月刚上市的美奇丝和巴黎欧莱雅专业美容美发产品。欧莱雅公关部有关人士强调，染发前必须先做皮肤测试，但目前国内许多消费者习惯上省略了这一步骤。

“合格产品不一定适用于所有人的体质。”专家认为，虽然产品在出厂前都必须经过检验，但消费者对化妆品的适用情况有个体差异。“对于美容美发机构推荐的产品，消费者可以要求做一下皮肤测试，可在手臂、大腿等人体局部皮肤进行试用。”

我们仔细分析三个使用XXX染发剂导致毁容的事件，就不难发现一个共同点，那就是受害人均是采用自己在家使用染发剂的方式染的发。并没有做过皮肤的测试，也没有在专家或者专业人士的指导下完成。

天气因素也提防

使用染发剂导致过敏的原因有很多，除了产品本身以及自身的原因外，连自然的天气的因素也都有可能成为惨剧的凶手。医学专家就曾指出，盛夏来临，高温天气时，无论使用多高档的染发剂都会伤害发质，还有可能伤到皮肤，都应谨慎对待。

天气炎热，人的皮肤本来就遭受着自然界的刺激，如强光照射、尘螨侵袭以及空气中其他污染物的刺激。加上天热出汗多，汗腺、皮脂腺等时刻处于紧张 的工作状态中，外来不良刺激进入皮肤的机会增多。在这种情况下，染发剂中的化学成分更易通过皮肤被人体吸收。此外，高温情况下，汗液中大量的代谢物质将使 过敏、发炎的皮肤更易滋生细菌，症状恶化。染发剂引起的皮肤炎症，表现为被刺激部位红肿、糜烂、流水、又痒又疼。

医学专家建议，高温天气时应谨慎染发。实在想染发，染发前应做相应皮肤试验，没有出现不良反应才可染发。染后的头发，要彻底清洗干净，同时不要使用劣质染发剂。当头皮有破损时，千万不要染发，以免感染化脓。

脱发-睡眠不足导致脱发

不良的生活方式是脱发的“催化剂”，因此要养成良好的生活及饮食习惯。

首先，每天睡眠不应少于7个小时，最好在晚上11点前入睡，否则会影响身体的神经内分泌系统，导致或加重原有的脱发。

染发、烫发应掌握好频率，一年别超过2次，两者应有时间间隔。这是因为，烫发剂和染发剂都会对头发最外层的毛小皮产生一定程度的损伤，导致毛鳞片的剥 蚀、缺损，发内成分的流失，使头发失去光泽、干枯易断。另外，吹风机吹出的热风会破坏毛干中的蛋白质，导致毛发中出现空洞，使毛发容易断裂，也应避免。

洗发本身是不会导致或加重脱发的。洗发的频率应因人而异，生活和工作环境很干净，头皮出油不多的人，可以每2天洗一次，而环境较脏或头皮出油多的人可每1天洗2次。需要注意的是，洗头时水温不要太高。

频繁梳头不会促进毛发生长，相反，一些不正确的梳头方式反而损伤头发。有些人为了让头发蓬松倒着梳头，会对毛鳞片造成损伤，从而导致头发断裂。应避免使用 尼龙梳子和头刷，因为静电会给头发和头皮带来不良刺激，最理想的是用软梳。另外，尽量不要在头发湿的时候梳头，因为这时头发含水量大，毛小皮比较粗糙，发 丝容易相互纠缠，如果强行梳理会导致头发断裂，损伤头发。

目前，中青年男性出现男性型脱发的非常多，一方面与遗传因素有关，但平时工作压力大、生活不规律是导致脱发提前出现和严重程度增加的重要因素。因此，预防脱发其实应该从改变自己的生活习惯开始。

人们平均每天掉 100根左右的头发 ,那是正常的 ,不足为奇。但当脱发远远大于此数 ,就应引起重视了。一般来说 ,常见的脱发有两种 :一种为斑秃 ,另一种为雄激素性脱发。

斑秃大多是由各种原因使毛囊血液供应减少 ,或者局部神经调节功能发生障碍 ,以致毛囊营养不良引起 ,但无毛囊结构破坏 ,因此经过治疗 ,新发还可再生并恢复原状。此种秃发多属暂时性的。

而雄激素性脱发发生原因可能往往与遗传和雄性激素有关。在男性青春发育期之后体内雄激素水平升高 ,雄激素睾酮使皮脂腺分泌皮脂 ,使毛乳头营养丰富 ,毛发生长期时间长 ,毛发生长良好。而人体内有一种 5-α还原酶 ,会使睾酮变成双氢睾酮 ,双氢睾酮使毛乳头血管收缩 ,毛乳头萎缩 ,变小 ,导致毛发脱落。它病程进展缓慢 ,最先从两侧额角处开始脱发 ,前发际线逐渐向后退缩 ,前额变高 ,对于此种脱发 ,现在也有了较好的治疗方法。

此外尚有其他性质的脱发 :

1、精神或身体的因素

精神压力过度是导致脱发常见病因。精神紧张、忧郁、恐惧或严重失眠等均能致使神经功能紊乱，毛细血管持续处于收缩状态，毛囊得不到充足的血液供应， 而头皮 位于人体的最上端，因而头发最易脱落。如果压力持续，再加上心理素质较弱，就易发生断发（毛）癖、食发（毛）癖、拔毛癖等。头部外伤，脊髓、延髓、中脑和 脑干的病变，均可以引起头发脱落。精神因素还会严重地影响头发的生长周期，长时间的视力疲劳、精神压力过重、神经过度紧张、急躁或忧虑情绪、熬夜等，均可 导致头发生长周期缩短，出现脱发现象，导致早秃。常见如产后脱发、重病后脱发、考试后脱发以及一些担负重大责任的单位负责人或商人的脱发。

2、激素分泌失衡及遗传因素

脑下垂体分泌的生长激素可促进头发的生长，而生长激素的缺乏则会使头发的生长速度相对变慢；性激素还会影响毛发的健美：女性激素使头发柔软而富有光 泽；男性激素则会使头发变得坚硬而粗壮，一旦体内性激素失去平衡，就可能出现毛发异常。因此，产后、更年期、口服避孕药等情况，在一定时期内会造成雌激素 不足而脱发；垂体功能减退、肾上腺肿瘤、肢端肥大症晚期等，均可导致头发的脱落。大部分女性在接近更年期时都会脱发，证实脱发的确可因荷尔蒙而致，如果是 这种情况可选择接受荷尔蒙治疗或由医生处方服食药物。

药物与头发的关系

许多药物会损伤头发，妨碍美容。免疫抑制剂、化疗药物、砷剂等药物可致脱发，长期服用抗菌素和止痛片，易出现头发细短、脱落的现象。部分营养补剂中含有兴奋剂，长期使用易使头发受损。普通的药物例如抗抑郁药、高血压药都可导致脱发，医生可建议病人转用别种药物。

其它的药物治疗因素如接受化学治疗、服用减肥药、胃溃疡药、维他命A酸；至于治疗癌病的化疗，很多人都知道会引致脱发，这是由于化疗会杀死癌细胞以外任何快速分裂的细胞，包括处于生长期的发根细胞，所以便会有约九成的头发脱落。当化疗停止后头发会逐渐重生生长。

4、食物及营养代谢的因素

由于偏食等因素而引起的营养不良，以及因消化不良、慢性消耗性疾病而致营养不均衡或吸收障碍，均可导致头发的正常生长被抑制而进入休止期，并出现头 发稀疏、枯焦、早白或脱落。食糖或食盐过量、蛋白质缺乏、缺铁缺锌、过量的硒等，以及某些代谢性疾病如精氨基琥珀酸尿症、高胱氨酸尿症、遗传性乳清酸尿 症、甲硫氨酸代谢紊乱等，也是头发脱落的原因。

营养素与头发的关系

碘可促进毛皮质中黑色素的形成，有助于细胞增生活跃，产生足量的黑色素，碘缺乏时黑发在生长过程会变成白色；

钙人体内缺钙时，头发会过度变粗、干燥且脆弱易断；

维他命缺乏叶酸、泛酸或B族维他命时，头发会变成灰白色；

蛋白质严重缺乏蛋白质的儿童，头发会慢慢变成红色；

金属元素缺锌会导致头发脱落，出现秃顶；缺铜、铁、钴则会使头发逐渐泛黄直至变白。

5、先天性的因素

先天因素如：发育缺陷、各种综合症、早老病等所引起的头发完全缺失或稀疏；毛囊发育不良造成头发细软易断。先天性秃发则是一种常染色体显性遗传病，多见于近亲结婚所生育的后代。

6、物理性及化学性的因素

生活环境与头发的关系

生活环境与头发的健美密切相关。现代城市中的黑烟和废气等严重污染了人们生活的空间，从而对头发产生危害；长期作业于强烈的阳光下，会造成头发的黑色素脱落而变成茶褐色，并可切断头发内的蛋白质供应，使发丝粗糙无光。

物理因素如拉伤、擦伤、烧烫外伤及电伤等；物理性因素造成的脱发：包括发型性脱发、局部摩擦刺激性脱发等机械性脱发；放射性损伤临床上划分为四度，均可引起头发脱落；日光中的紫外线过度照射，经常使用热吹风，头发也容易变稀少。

因物理因素造成的脱发又称物理性脱发。

化学因素如洗发精里的硼酸或染烫发药水：烫发剂、洁发剂、染发剂等美发化妆品是引起脱发的常见原因。近几年来，妇女头发变稀疏的发生率不断增多，可能与一些不负责任的广告宣传导致滥用洁发剂有关。因化学因素造成的脱发又称化学性脱发。

7、内分泌失调的因素

内分泌因素如甲状腺机能亢进；甲状腺过度活跃和不够活跃都可能导致头发变稀变薄，甲状腺补充剂可令头发回复正常。甲状腺素分泌量也与头发的优劣有关：甲状腺功能亢进时，头发较细软，功能减退时则头发干燥且无光泽。

8、免疫系统机能反应絮乱

免疫系统可能袭击毛囊，导致脱发，如自体免疫功能絮乱象红斑性狼疮造成的脱发；医生通常会在病人脱发的位置，直接注射少量类固醇。

9、护理不当及人为因素

人为因素与头发的关系不良生活习惯

（1）整天戴帽会压迫头皮血管，影响血液循环。尤其在炎热夏季，帽子妨碍通风，久之易使头发脱落；

（2）酗酒过度会导致胃肠疾病，从而影响蛋白质、维他命等营养素的消化与吸收；酒中所含防腐剂对毛发的健美也有很大的负面影响。

（3）经常饮用碳酸类饮料而对肠胃产生刺激，引起消化功能紊乱，影响营养素的吸收而导致脱发；

（4）过度的运动也会影响头发的健康，会出现毛丝柔细并导致脱发；

（5）长时间生活在空调房间，可致头皮屑增多和脱发。

护发与美发不当

（1）用刀削发尖易损伤毛髓质、毛皮质与毛表皮，常出现头发分叉与断裂现象，特别是削发后立即烫发，对头发损害更大。用剪刀剪断头发，不会损伤发端，头发才会健康地继续生长；

（2）烫发、染发不当会损伤毛皮质。烫发水能溶解毛皮质中的蛋白质与脂肪，使头发软化呈波浪状；染发剂则可去除毛发质内的黑色素，进而渗入染发剂中。因此烫发剂、染发剂的质量、使用的剂量与应用方法很重要，正确而恰当地使用可减轻其对头发的伤害；

（3）过多地使用电吹风会损害毛皮质与毛髓质，使头发干枯、分叉、变细直至脱落；

（4）发刷选择不正确和梳理方法不正确会损伤头发。可产生静电作用的尼龙制发刷，梳头时可产生高达数百伏特的静电，由毛干传至毛根下放电，导致毛孔 与毛根 组织之间产生气泡，久之，毛孔与毛根完全松离，而引起脱发；钢丝刷与猪毛制成的发刷会损伤头发，也不宜选用，最好选用橡胶或塑胶制的发刷或角质的木制的梳 子；

（5）洗头剂选用不当会影响头发健美。市面上出售的石油系列洗发剂、护发素，会使头皮的皮脂成分受损，导致皮屑增多、头皮干燥。

10、真菌感染、寄生虫、病毒及化脓性皮肤病等因素

感染性脱发：头部水痘、带状疱疹病毒、人类免疫缺陷病毒（HIV）、麻风杆菌、结核杆菌、梅毒苍白螺旋体，以及各种真菌引起的头癣均可引起脱发。局 部皮肤 病变如溢脂性皮肤炎、扁平苔藓、感染霉菌或寄生虫等也是造成脱发的主要因素。头皮上的嗜脂性真菌在皮脂腺分泌量大的前提下大量繁殖，成灾的嗜脂性真菌从毛 囊中获取营养并把代谢产物排放在那里，刺激毛囊和头皮出现慢性炎症－脂溢性皮炎，毛囊逐渐萎缩，生发功能逐渐减退，头发逐渐减少直至光秃。头螨是一种微小 的寄生虫，肉眼无法看见。它寄生在人类的毛囊里，以皮脂为食物，并且分泌解脂酵素（lipase），侵蚀皮脂腺，阻塞毛囊，造成脱发或秃头。

斑秃-鬼剃头

斑秃又称圆秃，是秃发(脱发)的一种，相当于中医所称的油风，俗称“鬼剃头”、“鬼舐头”。多发生于青壮年，表现为头部局限性斑片状脱发，呈圆形、椭圆形或地图状，大小不等，边界清楚，脱发处头皮光滑无炎症，而其周围头发易拔除。

秃发有斑秃、全秃、普秃三种。它们的病因及治疗大致相同，只是临床表现的程度不同而已。 少数患者数天或数月内头发全部脱落，称之为全秃；严重者可兼出现眉毛、腋毛、阴毛、毳毛脱落，称之为普秃。

斑秃是一种骤然发生的局限性斑片状的脱发性毛发病。其特点为病变处头皮正常，无炎症及自觉症状，常于无意中发现，头部有圆形或椭圆形 脱发斑，秃发区边缘的头发较松，很易拔出，斑秃的病程经过缓慢，可持续数月至数年。本病可自行缓解又常会有反复发作。斑秃中约有5—10% 的病例在数天内或数月内头发全部脱光而成为全秃（alopecia totalis），少数严重患者可累及眉毛，胡须，腋毛，阴毛等，全部脱光称普秃（alopecia universalis）。

斑秃是一种局限性斑状脱发，骤然发病，病因由神经精神因素引起毛发生长的暂时性拟制，内分泌障碍，免疫功能失调，感染或其他内脏疾患的可能。头部突 然出现圆形或椭圆形斑状脱发，患处头皮光滑发亮。病情进展时则损害扩展，周围毛发松动易脱，个别患者头发可能全部脱光，严重时眉毛、胡须、腋毛、阴毛等亦 可脱落。

斑秃可自愈，但经常复发，病程可持续数月或更久。恢复期的新发呈纤细柔软灰白色，似蠢毛状，逐渐粗黑，最后恢复正常。对于斑秃的治疗，应寻找病因，然后采用心理疏导等方法进行心理治疗，还可辅以内服及外用的药物治疗。

斑秃往往是突然发生一片或秃发斑，大多数人没有任何自觉症状，常常是别人无意中发现或理发时才被告知。这种秃发斑多数为圆形或椭圆形，边缘非常整 齐，如果 秃发斑较多时其分界则不很清楚。刚刚出现时斑块较小，直径1-2厘米，可逐渐扩大。在斑秃发展扩大时，周缘看似正常的头发很容易被拔掉，头发根很细，呈灰 白色，没有痛感。局部头皮为正常颜色，表面光滑发亮。仅有很少病人在刚刚发病时伴有局部头皮瘙痒，斑秃处头皮微红。

有些斑秃病人的秃发斑较小，片数也只有1-2片，在几周内即停止扩大，周边部的头发长得很牢固，没有毛发松动症状。这是好转的现象，很快光滑秃斑 上出现纤细浅色毳毛，逐渐变黑变粗变密而头发恢复正常。有些斑秃，周边头发松动明显，扩大较快，并陆续发生新的秃发斑，虽然其中几片斑秃中央出现了一些新 生长的浅色毳毛，但边缘处看不到这种新生细毛，这种斑秃也不会在短时间内痊愈。斑秃一般均先发生于头皮，但在罕见情况下可先出现于眉毛、胡须或四肢毳毛处 也是边缘清楚的脱毛斑，往往不容易在短时间内消失。如患者，已有白发，则斑秃处黑发先脱落，白发晚脱或不脱。

儿童除发生如成人一样的斑秃外，还可在枕下沿后发际线头发脱落，称蛇形脱发，很难治愈。

斑秃痊愈后常可复发。有的患者几个月后复发，也有几年后又复发。有的病人只复发1次，有的复发多次。斑秃的复发没有规律，一般来说有斑秃家族史者， 尤其是有斑秃复发家族史者出现复发的机会要大的多。少数患者斑秃多次复发后，可发展成全秃后普秃。少数斑秃患者可出现指甲的改变，表现为甲板上多数粟粒大 小的凹陷甲板表面粗糙不平以及脆甲、脱甲等。伴有指甲改变者是斑秃严重的标志。大部分轻症和一般性斑秃部出现指甲异常，伴有甲板病变者，斑秃的自愈可能性 较小。

斑秃的病程很不一致，斑秃会发展到什麽程度，是否会出现新的损害，是否会有复发等很难做出准确的预测。一般情况下，多发性斑秃比单发性者重些，全 秃比斑秃更重，最重者为普秃。

斑秃患者不要过分担心

斑秃的症状 斑秃大多突然发生，由于没有任何自觉症状，患者自己不知道已患了斑秃，常常由别人无意发现或理发时才被告知。患者头部突然出现斑状脱 发，边缘清晰，头皮光亮。病情进展时损害逐渐扩展，斑秃边缘的毛发松动易脱；个别患者的头发会全部脱落，这种情况称为全秃；严重 时眉毛、胡须、腋毛、阴毛等亦会脱落，这种情况称为普秃。

男性斑秃患者中大约有10%的人可能发展成普秃。斑秃大多能自然痊愈，但常常会复发，病程可以 持续几个月或者更久。恢复期的新发纤细、柔软、灰白色、似毳毛状，以后逐渐变粗、变黑，最后恢复正常。

据统计，在发生斑秃前有情绪紧张者占半数以上，而伴有失眠、多梦者则更多。当人们受到各种精神因素刺激时，机体的内分泌功 能发生紊乱，免疫系统功能降低，导致体表的毛发生长出现暂时性抑制，使得毛发的生长基地：毛乳头出现一时性的血液循环量减少，局部缺血、缺氧，毛发生长所 需的养料不足，毛乳头萎缩，于是便发生了脱发。但是也有的人没有明显的精神因素，有的斑秃有遗传性，这从医学上来讲就是与自身免疫有一定的关系。

斑秃不可怕 斑秃尤其是全秃、普秃，会给患者带来较大的痛苦，心理的痛楚更是苦不堪言。患者四处求医问药，他们的急切心情可以理解。不过，斑秃患者 应该 知道这样一个事实：斑秃这种脱发是有自限性的，它的病程虽然可持续数月、甚至数年之久，但大多患者可以完全或部分自然痊愈。因此，斑秃患者 要保持乐观态度，保持良好的精神状态，不要紧张、焦虑，对治疗要充满信心，消除脱发带来的消极情绪及精神负担。要知道，疾病的恢复都有一个过程，斑秃的病 程多半要持续几个月以上，头发是逐渐长长的，如果整天沉浸在严重精神负担之下，就可能影响和妨碍新发的生长，导致斑秃的自愈时间延缓，甚至会造成恶性循 环，加重脱发病情。
注意生活起居 斑秃患者在日常生活调养中应注意精神、休息和饮食三个方面。

（1）解除思想顾虑 增强治 愈的信心，保持心情舒畅，缓解紧张的精神压力。斑秃虽然脱发严重，病程进展快，但大多数是能够治好的，只是治疗时间较长，至少4-6个月。因此完全没有必 要终日紧张、忧虑不安。要知道，紧张忧虑的情绪只会加剧血管舒缩功能的失调，使毛囊供血障碍加重，不利于头发的再生，反而会加重病情。

（2）合理安排作息时间 养成良好的生活习惯，注意劳逸结合，不要过度用脑和熬夜，保证充足的睡眠时间。

（3）饮食的调理也很重要 治疗期间要忌吃辛辣刺激性食物，不要喝浓茶与咖啡，特别是临睡前不喝刺激性饮料，以免影响睡眠和休息，加重脱发；在日常 饮食 中应注意补充B族维生素和蛋白质，如瘦肉、蛋类等；胱氨酸是头发生长的重要成分，也可多食富含胱氨酸的食品，如豌豆、猪肉、鸡肉、蛋黄等。

一般说来，脱发并不严重，但是在毛发再次生出以前，它会让你的孩子感到为难。先检查孩子头皮部脱发的区域，如果局部为粉红色或灰色，并有鳞屑，则表 明是癣。如果不是，则孩子可能是患了斑秃。注意孩子是否有精神性不自主的动作，在注意力集中时揉搓或拉扯头发。不要为脱发而大惊小怪,将此事告诉孩子的老 师及同学，以免使孩子在外受到嘲笑。

The average human head has about 100,000 hair follicles. Each follicle will grow an average of about 20 individual hairs in a person’s lifetime. Average hair loss is about 100 strands a day.

Incidence of pattern baldness varies from population to population based on genetic background. Environmental factors do not seem to affect this type of baldness greatly. One large scale study in Maryborough, Victoria, Australia showed the prevalence of mid-frontal hair loss increases with age and affects 57% of women and 73.5% of men aged 80 and over. According to Medem Medical Library’s website, male pattern baldness affects roughly 40 million men in the United States. Approximately 25 percent of men begin balding by age 30; two-thirds begin balding by age 60.

Male pattern is characterized by hair receding from the lateral sides of the forehead, known as “receding hairline”. Receding hairlines are usually seen in males above the ages of 25 but can be seen as early as mid-teens.

An additional bald patch may develop on top (vertex). The trigger for this type of baldness (called androgenetic alopecia) is DHT, a powerful sex hormone, body, and facial hair growth promoter that can adversely affect the hair on the head as well as the prostate.

The mechanism by which DHT accomplishes this is not yet understood. In genetically-prone scalps, DHT initiates a process of follicular miniaturization. Through the process of follicular miniaturization, hair shaft width is progressively decreased until scalp hair resembles fragile vellus hair or “peach fuzz” or else becomes non-existent. Onset of hair loss sometimes begins as early as end of puberty, and is mostly genetically determined. Male pattern baldness is classified on the Hamilton-Norwood scale I-VII.

It was previously believed that baldness was inherited from the maternal grandfather. While there is some basis for this belief, both parents contribute to their offspring’s likelihood of hair loss. Most likely, inheritance is technically “autosomal dominant with mixed penetrance” (see ‘baldness folklore’ below)

There are several other kinds of baldness:

• Traction alopeciais most commonly found in people with ponytails or cornrows who pull on their hair with excessive force.
• Trichotillomania is the loss of hair caused by compulsive pullingand bending of the hairs. It tends to occur more in children than in adults. In this condition the hairs are not absent from the scalp but are broken. Where they break near the scalp they cause typical, short, “exclamation mark” hairs.
• Traumas such as chemotherapy, childbirth, major surgery, poisoning, and severe stress may cause a hair loss condition known as telogen effluvium.
• Worrisome hair loss often follows childbirth without causing actual baldness. In this situation, the hair is actually thicker during pregnancy due to increased circulating oestrogens. After the baby is born, the oestrogen levels fall back to normal pre-pregnancy levels and the additional hair foliage drops out. A similar situation occurs in women taking the fertility-stimulating drug clomiphene.
• Iron deficiencyis a common cause of thinning of the hair, though frank baldness is not usually seen.
• Radiation to the scalp, as happens when radiotherapy is applied to the head for the treatment of certain cancers there, can cause baldness of the irradiated areas.
• Some mycotic infections can cause massive hair loss.
• Alopecia areatais an autoimmune disorder also known as “spot baldness” that can result in hair loss ranging from just one location (Alopecia areata monolocularis) to every hair on the entire body (Alopecia areata universalis).
• Localized or diffuse hair loss may also occur in cicatricialalopecia (lupus erythematosus, lichen plano pilaris, folliculitis decalvans, central centrifugal cicatricial alopecia, postmenopausal frontal fibrosing alopecia, etc.). Tumours and skin outgrowths also induce localized baldness (sebaceous nevus, basal cell carcinoma, squamous cell carcinoma).
• Hypothyroidismcan cause hair loss, typically frontal, and is particularly associated with thinning of the outer third of the eyebrows (syphilis also can cause loss of the outer third of the eyebrows)
• Hyperthyroidism can also cause hair loss, which is parietal rather than frontal.
• Temporary loss of hair can occur in areas where sebaceous cysts are present for considerable duration; normally one to several weeks in length.

There is no consensus regarding the details of the evolution of male pattern baldness. Males with facial hair and those with bald or receding hair were rated as being older than those who were clean-shaven or had a full head of hair. Beards and a full head of hair were seen as being more aggressive and less socially mature, and baldness was associated with more social maturity. A review of social perceptions of male pattern baldness has been provided by Henss (2001) .
